Note that the reason Unix (and thus POSIX) filesystems are case-sensitive is probably not due to a specific design choice by the the authors of Unix back in the early '70s, but that they didn't want the extra complexity of implementing this, especially since simplicity of code was a design goal of the system ("New Jersey style" instead of "MIT style").
